• Presentation title and presentation style (Oral/Poster) if you have any preference (N/A if you do not give a talk)
• Travel information (if you would like to stay at the Ala moana Hotel, please tell us your arrival and departure dates, the number of accompanying person and room type (2 rooms, Twin room, Triple room, etc. (if any).)
• Travel information (if you would like to take group booking flight, please tell us JAL or ANA, copy of your passport page with your portrait and the mileage number.)
• Please check the status of your ESTA.
